When submitting a claim by mail or email, you will need to include a completed claims form. CLICK HERE to get our claims form.

Please provide the following with your submission:

• -Brief description of the facts and circumstances surrounding the claim
• -A copy of the title policy
• -Any supporting documentation
• -Your contact information
• -Pending deadlines, if any
